Clinic Policies

Appointments and Fees:

We require a credit card on file to hold your scheduled appointments. Please complete the enclosed credit card authorization. This card will be used for your Monthly Membership Fees if you sign up for a Membership, any a la a carte appointment fees and any other charges to your account.

During your first visit, your Citrin Longevity naturopathic doctor will talk with you in depth about your health concerns, pertinent past medical history, etc. and determine if you would be a good fit for one of our Membership plans or continued a la carte services.

Please note that fees for appointments and Membership Fees do not include the cost of any supplements, medications or laboratory tests, and all fees are subject to change.

Cancellation, Rescheduling and Missed Appointments:

Your appointment times are reserved especially for you. If you cannot make your scheduled appointment, please inform us as soon as possible so that we may give this time to another patient. Failure to provide us with at least 24 hours (one business day) advance notice of appointment cancellation and scheduling changes will result in a non-refundable charge to your credit card on file of $100. If you miss your appointment without notification, your credit card on file will be charged for the full amount of the scheduled appointment.

Payments; No Insurance:

Payment is due in full at the time of service unless a prior arrangement has been made with the office. We accept VISA, Discover, MasterCard and American Express credit cards. Citrin Longevity cannot and does not accept or bill insurance, Medicare or Medicaid for any services provided, and our office does not submit claims for services on your behalf. Upon request, we will provide you with a statement showing your payment and a coded superbill so that you may submit it to your insurance company (not Medicare) for possible reimbursement at an out-of-network rate depending on your individual plan. Superbills cannot be provided for Membership Fees or visits included in a Membership.

 

Lab Tests:

We offer a variety of labs, both conventional and specialty, based on patient need. Fees for lab tests are handled between the patient and lab, with the exception of certain labs where we can pay on the patient’s behalf. In such case, patients must submit payment to our office as soon as the lab is ordered. Insurance may cover some labs, depending on your individual plan, but if you choose to use your insurance rather than pay the lab company directly, we cannot guarantee what your insurance may cover or what your out-of-pocket costs may be. It is the patient’s responsibility to determine lab coverage with your insurance carrier. What you might owe for lab tests and what is covered by insurance varies widely amongst networks. To prevent the stress of unexpected bills, if you choose to use your insurance, we recommend you contact your insurance company to understand your lab benefits prior to completing any lab tests.

Phone Consultations and Questions:

During all phases of your care, it is important that you fully understand your treatment program. Should you have questions regarding your treatment or progress, we suggest that you make an appointment so that we may properly meet your needs.

Emergencies and Urgent Matters: For medical emergencies, please call 911. We do not treat emergencies in office. If you have an urgent situation, proceed to urgent care. If you have an urgent question, you can call our office during business hours, and if possible, one of our naturopathic doctors will call you back. However, we cannot guarantee a time frame for that return. We will do our best to get back to you quickly.

Notice that Services Are Not Primary Care:

The Citrin Longevity naturopathic doctors do not act as your primary care provider, and the services provided by Citrin Longevity are health optimization services that are in addition to, and not a replacement for, the care of your primary care provider (PCP).  Responsibility for your overall medical care should remain with your PCP. If you do not have a PCP, we strongly encourage you to locate one for services that Citrin Longevity does not provide and for other urgent health matters, after-hour health needs, on-call physician access and emergencies.

Notice to Cancer Patients: We do not treat cancer. We can assist in health optimization while you are working in conjunction with an oncologist.

Notice Regarding Pregnancy and Breastfeeding: All patients who are pregnant or breastfeeding must inform their Citrin Longevity doctor if they know, suspect or are pregnant or are breastfeeding, as some of the therapies could present risk to the pregnancy, fetus or nursing baby.
